an old military pilot term. When you go full throttle, you are pushing the levers (topped with round grips like balls) all the way forward, against the control panel (wall). You know, like going "pedal to the metal" in a car.

Balls to the wall = going all out.

No better way to describe the way Caldwell wrestles almost every time I've seen him.
